Form Fillable PDF Playbooks?

Hiya gang,
I'm collating some custom playbooks, and my PDF-fu is crap. I just want a 'blank' playbook proforma that I can manually fill in, or a form fillable electrickery one I can do on the puter. Anyone have one?
