Title: Mystery sheet as a form

I was wondering, has anyone made a form of the mystery sheet with menus and such? It would be a great help for people like me who can't read their own handwriting :-)

I don't know much about the ways of PDF forms, but I can have a look into it. It might be a webpage that generates a PDF is easier, hmm...
